The Augustine Project
Consent and Details
Fill this in the same day, while the names are still fresh. It takes three minutes and it is the difference between a recording and an archive.
Free to print, copy and adapt.
Photocopy this card for your parish or your family. If your diocese has its own consent wording, use theirs instead — the point is that something is signed, not that this particular page is.

Sensitive material
Some recordings contain family conflict, accusations, medical detail, or stories about people who are still living. Note here anything that should not be shared outside the family without asking first.
Copies
The narrator and the family should keep the original, unedited recording and at least one independent backup, in two different places, and should keep the original whenever an edited clip or a transcript is made from it. Do not rely on any website, including this one, as the only home of an irreplaceable voice.
